A multicenter cohort of patients with sustained dream loss following lesions will be established, targeting approximately 50–100 cases, and paired with 100–200 patients without dream loss but with similar lesion load and location as controls; at baseline, 3, 6, 12, and 24 months, multi-night REM and NREM scheduled awakenings, polysomnography, dream recall, and general memory assessments will be conducted, tracking survival, metabolism, and daytime functioning; project duration approximately 4–5 years. This natural experiment can test whether sustained dream loss is accompanied by independent physiological impairment, but existing methods cannot fully exclude unreported dream experience, nor attribute all effects of lesions to dream loss.
