In approximately 450 patients with persistent somatic symptoms or chronic primary pain, first predefine symptom-related roles, avoidances, and life goals through independent interviews, without inferring purpose from treatment outcomes; randomly assign to replacement-goal and role-reconstruction, symptom-management CBT, or time-matched support, with 12-week intervention and 18-month follow-up. Each month record original symptoms, new symptoms, function, and medical diagnoses; test whether role reconstruction precedes relief and whether new symptoms exceed the control background rate when role replacement is absent; 'sufficient desire' must also be predefined with scales and thresholds.

Specificity of Seth's formulation
