Conduct a multicenter RCT of approximately 4,000 to 6,000 coronary heart disease patients, all receiving guideline-based treatment; randomly assign to core-belief CBT, time-matched supportive intervention, or usual rehabilitation, with 6 to 12 months of intervention and 5-year follow-up. Primary endpoints are blinded-adjudicated cardiovascular death, nonfatal myocardial infarction, and stroke; repeatedly measure core beliefs, adherence, exercise, and depression to distinguish cognitive changes from behavioral pathways. Specificity of Seth's formulation
CBT acknowledges that cognition affects distress, coping, and behavior but does not presuppose that all illness must be cured through cognitive change; 'recurrence in different forms otherwise' is an additional prediction that requires predefining which new diseases count as recurrence and the observation window.
